Sunbeam Developmental Resource Centre – Waterloo Region (SDRC) is the single point of access to developmental services for children in the Region of Waterloo. SDRC provides service coordination and clinical services to children, youth and adults with Intellectual Disabilities and/or an Autism Spectrum Disorder (ASD), living in Waterloo.

We are presently inviting applications for the position of:

Urgent Response Service (URS) Coordinator –  Full-Time

The primary hours of work are 8:30 a.m. – 4:30 p.m. Monday – Friday but the successful candidate may be required to work some evenings and weekends.

The Urgent Response Services program provides time-limited services and supports for up to 12 weeks. The program is intended to respond rapidly to a specific need for children and youth up to 18 years of age and registered in the Ontario Autism Program (OAP), with the purpose of preventing further escalation of risk of harm to self, others and/or property. Each child or youth eligible for the program will have a unique URS Plan developed in consultation with service providers who will be providing the direct service to the child or youth. The plan is reviewed at 4, 8 and 12 weeks to ensure goals are on track to be met. After the 12-week plan has been completed, the family is provided with a transition plan with any necessary referrals made to ensure the gains made in the program can continue after the service ends.

The successful candidate will have experience:

  • Contacting clients and families within 48 business hours of referral
  • Experience assessing and determining eligibility for URS including appropriate level of service within service navigation, interprofessional collaboration, direct service, and respite
  • Initiating internal and external referrals in consultation with family
  • Supporting the development, monitoring, and updating of the URS Plan in collaboration with the child/youth and family
  • Facilitating the collaboration of relevant parties to refine and coordinate the delivery of the URS service elements
  • Providing leadership and support to the development and use of an inter-agency, interprofessional and crosssectoral URS plan that addresses the single area of urgent need for the child/youth and family
  • Developing supportive working relationships with clients and their families
  • Developing and maintaining strong working relationships with service providers and agencies involved in client care
  • Ensuring the coordination of URS with existing services and supports to promote the transfer of knowledge prior to and at the transition out of URS
  • Working collaboratively with other service navigation supports including IIO Care Coordinators and Service Planning Coordinators when appropriate

Qualifications:

  • University degree in a related health care field or Social Service Worker diploma, Masters of Social Work preferred.
  • 3-5 years of experience with children and youth with Autism
  • 2-5 years of experience in service coordination required
  • Sound knowledge of local community and social services
  • Good working knowledge of provincial policies and legislation concerning children and youth
  • Experience working in collaboration with service providers across sectors (e.g. education, health, community supports)
  • Experience working with caregivers of children with complex needs
  • Strong verbal and written communication skills
  • Ability to work independently
  • Exceptional interpersonal skills
  • Computer proficiency with MS Office & Meditech
  • Bilingualism in French and English is an asset
