    Sunbeam Developmental Resource Centre – Waterloo Region (SDRC) is the single point of access to developmental services for children in the Region of Waterloo. SDRC provides service coordination and clinical services to children, youth and adults with Intellectual Disabilities and/or an Autism Spectrum Disorder (ASD), living in Waterloo Region.     The TAY Coordination Program is a program of Sunbeam Developmental Resource Centre – Waterloo Region (SDRC). The TAY Coordination Program supports individuals who are transitioning from Central West Region CASs Extended Care services, if eligible for Adult Developmental Services (DSO).

    The primary hours of work are 8:30 a.m. – 4:30 p.m. Monday – Friday but the successful candidate may occasionally work adjusted hours when having meetings with individuals supported, families, and agencies supporting the individual when as required. Travel is required throughout Central West Region and occasionally beyond, to complete monthly visits with the individuals on the TAY Coordinator’s assigned caseload, at the individual’s residential placement.

    The successful candidate will have experience:

      • With direct case management and oversight
      • Working with individuals with an intellectual disability, Dual diagnosis, and FASD
      • Working with youth and adults 16-30 years of age
      • Coordinating services for Transitional Aged Youth in Outside Paid Residential Resources
      • A sound working knowledge of the Child Welfare System
      • Familiar with working with individuals involved in the Justice System
      • A comprehensive understanding of Developmental Services Ontario
      • Presenting individuals to be considered for permanent placement in MCCSS Transfer Payment Agencies
      • Budget management and oversight
      • Working knowledge of Quality Assurance Measures (QAM) required in group homes and Host Family Homes
      • Experience with various levels of Behaviour Support Plans and their appropriate implementation
      • Ensuring Individual Support Plans have captured an individual’s support needs, goals, and dreams and can be implemented in the residential setting they are currently living

    Qualifications:

      • Degree in Social Services or a related field
      • Familiar with the Services and Supports to Promote the Social Inclusion of Persons with Developmental Disabilities Act/Child and Family Services Act/Personal Health Information Act/and various other federal and provincial legislations, directives and guidelines, corporation bylaws, policies, and procedures
      • Familiarity with the special challenges presented when an individual with complex needs including Dual Diagnosis, Autism, and FASD
      • Proven advocacy experience working with Supervisors and Managers at support agencies, direct support staff, schools, medical professionals, families, and individuals supported
      • Excellent consulting and oral/written communication skills
      • Two years of experience using a consultative model in a community setting
      • Emergency First Aid and CPR required (or must be prepared to obtain)
      • Valid driver’s license and reliable transportation are required for travel throughout the Central West Region
