Intake Resource Coordinator (approx. 15-month contract)- Mississauga
Full-Time (37.5 hours/week)
Start Date: October/November 2025
The Intake Resource Coordinator is the point of first contact for MCCSS adult developmental services and supports in the Central West Region. Responsible for confirmation of eligibility, referral for application package, and processing and maintaining applicant records for adults who have a developmental disability and/or dual diagnosis (mental health and developmental disability) who are seeking MCCSS funded developmental services.
Specific job duties include, but are not limited to, the following:

- Provide applicants and respondents with information regarding MCCSS services and supports which can be applied for via the application process
- Obtain and review applicants information to confirm eligibility as per MCCSS requirements, directing as needed to community resources to obtain documentation
- Communicate eligibility outcome to applicants/families by letter
- During eligibility process if applicant identifies urgent need, complete referrals to service coordination and/or service solutions
- Process requested referrals through DSCIS
- Upon contact review and ensure individual’s information is up to date in agency and ministry databases processing any new referrals and requests
- Ongoing maintenance of outcome plan in DSCIS
- Actively participate on internal and external committees as required
- Maintain all necessary documentation and records in DSCIS and EMHWare
The successful applicant will have:

- A university degree or college diploma in a related social services field (Sociology, Psychology, Developmental Services, Community Services, etc.) combined with experience serving individuals of all ages with a developmental disability, including those who have a dual diagnosis in a variety of community settings and programs
- Demonstrated understanding and respect for people of diverse backgrounds
- Excellent verbal and written communication skills
- Strong consulting skills
- Proficiency with MS Office and other computer related equipment and programs
- Ability to work independently and as part of a multi-disciplinary team
- Possession of a valid vehicle operator’s license and clean driving record
- Certification in CPI-Prepare Training would be an asset
- Require some flexibility including evenings and occasional weekends
