Sunbeam Community & Developmental Services with over 500 employees is a large non-profit, multi-division, multi-site, organization that is committed to serving individuals living with complex developmental, intellectual, behavioural, and/or physical disabilities.

Developmental Services Ontario Central West Region, administered by Sunbeam Community & Developmental Services, is the contact point for adult developmental services in Dufferin, Halton, Peel, Waterloo, and Wellington.

The role of the Assessor is to complete application packages, and Support Intensity Scales for identified adult clients who have a developmental disability and/or dual diagnosis (mental health and developmental disability) in Central West Region. To complete any necessary training, certification, and documentation needed to ensure thorough understanding of individual needs and goals in accessing developmental services. To work within processes and guidelines established, particularly regarding consent and communication. To assist in matching individual needs and goals to developmental and community services/supports. To working with, and mentoring volunteers

Specific job duties include but are not limited to the following:

    • Administer the Application for Adult Developmental Services and Supports (ADSS) and Supports Intensity Scale Adult Version (SIS-A) with applicant and respondents as per MCCSS training and guidelines and business standards
    • Compile an Assessor Summary Report (ASR) via MCCSS template completing within MCCSS guidelines and business standards
    • Ensure uploading of all documentation into MCCSS Developmental Services Consolidated Information System (DSCIS)
    • Ensure completion of referrals in both DSCIS and EMHWare and all other documentation completed in DSCIS

The successful applicant will have:

    • A university degree or college diploma in a related social services field (Sociology, Psychology, Autism, Developmental Services, Community Services) combined with five (5) years of experience serving individuals of all ages with a developmental disability in a variety of community settings and programs
    • Ability to complete MCCSS mandated foundation training and ongoing quarterly video training to administer the Application for Developmental Service and Supports package
    • Pass the MCCSS Interviewer Reliability Qualifications Review (IRQR) to administer the Supports Intensity Scale (SIS-A). Maintain designation of Qualified Ontario Developmental Services Application Assessor by successfully achieving recertification every 12-18 months
    • Demonstrated understanding and respect for people of diverse backgrounds
    • Excellent verbal and written communication skills
    • Strong consulting skills
    • Proficiently with MS Office and other computer related equipment and programs
