Developmental Services Ontario Central West Region, administered by Sunbeam Community & Developmental Services, is the contact point for adult developmental services in Dufferin, Halton, Peel, Waterloo, and Wellington.

DSO Central West Region is presently inviting applications for:
Service Navigator
Temporary Full-Time (37.5 hours/week)
13 Month Contract

The role of Service Navigator is responsible for the provision of navigation services within the Developmental Services Ontario Central West Region program. The Service Navigator will collaborate with individuals, their families and staff of community agencies to address the needs of adults who have a developmental disability and/or dual diagnosis who are seeking MCCSS funded developmental services. Specific job duties include but are not limited to the following:

  • Provide information to individuals and the public about how to apply for MCCSS funded adult developmental services and supports
  • Responsible for DSO general email inquiries
  • Assist individuals with developmental disabilities and their families/caregivers by providing information about MCCSS services and resources and/or non-MCCSS services and resources
  • Upon contact review and ensure individual’s information is up to date and accurate
  • Maintain ongoing contact with individuals, families, and staff at community agencies through a variety of methods (e.g. follow up phone calls, resource fairs, presentations, organized drop in’s, etc.)
  • Establish and maintain both formal and informal networks both across and within sectors through interagency committees and collaborative initiatives
  • Work in collaboration with the community to inform and direct people to available community supports, fee-for-service options, other government supports, employment, etc.
  • Responsible for organizing community events calendars for specific communities as a resource for families
  • Actively participate on internal and external committees and working groups, always representing the best interests of the organization and the individuals that we support
  • Facilitate data collection when necessary
  • Ensure uploading of all documentation into MCCSS Developmental Services Consolidated Information System (DSCIS)
  • Maintain program statistics for purposes of evaluation and research
  • Maintain all necessary documentation and records in DSCIS and EMHWare
  • Participate in meetings as required
  • Undertakes other assigned duties, as required

The successful applicant will have:

  • A university degree or college diploma in a related field (Developmental Service Worker, Autism Behavioural Sciences, Psychology, Sociology etc.) supplemented by several years of broad-based experience serving individuals of all ages with a developmental disability, including those who have a dual diagnosis; extensive experience in service provision with developmentally challenged individuals in a variety of community settings and programs
  • Excellent verbal/written communication skills
  • Ability to work independently and as part of a multi-disciplinary team
  • Possession of a valid vehicle operator’s license and clean driving record
  • Certification in CPI-Prepare Training would be an asset

Hours of work will typically be Monday to Friday 8:30am – 4:30pm (37.5 hours in a week) but will require some flexibility including evenings and occasional weekends.  Hours may exceed thirty-seven and a half (37.5) in a week.
